In our effort to provide high quality patient centered care, we are honored to seek recognition as a Patient Centered Medical Home (PCMH) through the National Committee for Quality Assurance. As your medical home, Tri-State Community Healthcare Center provides accessible, continuous, comprehensive, patient-centered, coordinated, compassionate, and culturally effective care. We are committed to coordinating your treatment through your primary care provider and/or care management team to ensure you receive the most up-to-date, evidence-based care.
Working in a team-based approach, we offer whole-person care, evidence-based preventive and wellness care for diabetes, and other chronic conditions, care when you are sick and education and self-management support.
It is the practice of Tri-State Community Healthcare Center to inform patients and families about the Patient Centered Medical Home (PCMH) model of care, it’s role in your care and the responsibilities of patients and families as partners in their healthcare. When scheduling a new patient appointment or annual physical, the front office staff is responsible for confirming the patient’s Primary Care Provider (PCP). When new patients arrive for their appointment, front office staff will provide patients with an orientation packet to complete along with a copy of the Patient Centered Medical Home brochure which includes a list of services that we offer, and information about their primary care provider and Tri-State Community Healthcare Center.
At least annually, patients are reminded and offered PCMH education via a brochure, other printed handout or directed to the website [or if you have a patient education screen in the waiting room or exam rooms that rotates patient reminders, health messages, education, etc, you could include PCMH details digitally for viewing live while at your health center locations]. While clinical information is getting updated on follow-up visits by the nursing staff, the patient and clinic role in PCMH shall be reinforced, reminded, and encouraged including patients’ responsibility in the care team. If the patients have any Care Coordination issues, the staff will also visit with the patient and provide education and/or referrals to community resources when applicable. The patient’s PCP will treat patients utilizing evidence-based guidelines. Any questions or concerns patients may have will be shared with the patient’s care team and addressed.
